New Patients
Appointments for new patients take about four to six weeks. If you are a new patient to this practice:
- Arrive approximately 15 minutes early for your appointment. This will allow time for the receptionist to review your insurance documentation and enter you into our computer database.
- Include accurate insurance information and the name of your referring and primary care physicians.
- Accurately list any medical problems, medications, dosages and medical allergies. By carefully completing this form you will minimize the risk of having a dangerous drug interaction as a result of any treatment that your doctor might prescribe.
- Please bring any relevant xrays, sleep studies, or medical records with you.
- It is also important that you bring your insurance card, as well as some type of identification, such a driver's license.
These forms may be completed on-line and then printed, or printed first and completed by hand. You may E-mail the form to us in advance, but please be sure to bring the form with you for your first visit. Please see the security statement regarding Email.
Nurse Practitioner Appointments
Because of the large number of patients waiting to see Dr. Armstrong, most new patients are seen first by a nurse practitioner who specializes in ear, nose and throat disorders. She will participate with Dr. Armstrong throughout your health care in his office, and be available to you even when he is at surgery. She is an excellent resource for education regarding your condition and treatment, and will help to maintain a more personal approach to your care.
